Notes:
1. This form must be completed and forwarded to WSI National Office at least 7 days in
advance of the first day of the course.
2. When courses are run independently of educational institutions and WSACs, the Tutor /
Organiser must inform the local Water Safety Area Committee – details on the WSI web site.
3. All Tutors must be WSI members
